Running’s True Impact: Debunking Myths on Knees and Ageing

The prevailing belief that running inevitably damages knees is largely a misconception, according to scientific evidence. Far from causing long-term harm, regular running appears to strengthen knee joints and improve overall bone health, actively working against degenerative conditions. This challenges a long-held myth that has deterred many from embracing a highly beneficial form of exercise, suggesting instead that the human body is designed to adapt positively to physical stress.

Running is indeed a high-impact activity, with each footfall generating a force equivalent to two to three times an individual’s body weight. The knees bear a significant portion of this load, absorbing three times more stress during running than during walking. However, this mechanical stress is not detrimental; rather, it acts as a crucial stimulus for the body’s dynamic systems, promoting growth and adaptation within bones and cartilage, much like resistance training strengthens muscles.

The knee joint, an incredibly robust structure, relies on movement and appropriate load to maintain its integrity. Cartilage, the flexible connective tissue cushioning the bones, requires mechanical stimulation to thrive. Research indicates that periods of inactivity, such as prolonged bed rest, lead to the deterioration of bone and cartilage. Conversely, the controlled impact of running facilitates nutrient flow into cartilage, a process vital for its health and ability to adapt and become stronger over time.

Studies consistently show that runners typically exhibit thicker knee cartilage compared to non-runners, a finding that directly refutes the notion of wear and tear. Furthermore, runners generally possess superior bone mineral density, indicating stronger bones. Some research even suggests a protective effect against the development of osteoarthritis, a common age-related joint condition, with increased running potentially correlating with greater protection, although further investigation is warranted in this specific area. These findings underscore running’s positive contributions to musculoskeletal longevity.

Concerns about starting running later in life are also being addressed by emerging research. While direct studies on older adults initiating running are limited, related investigations offer strong indications of safety and efficacy. A 2020 study, for instance, found that individuals aged 65 and older who engaged in high-intensity plyometric training—an activity involving joint loads significantly higher than running—experienced improvements in strength and function without adverse effects, suggesting that a gradual introduction to running would also be safe and beneficial for older populations.

The persistence of running’s negative reputation often stems from the high incidence of injuries among runners, with knee injuries being particularly common. However, the vast majority of these are “overuse” injuries, primarily resulting from poor load management rather than the act of running itself. These occur when individuals increase their running intensity or distance too rapidly, failing to allow their bodies sufficient time to adapt and strengthen in response to the new demands placed upon them.

To mitigate injury risks, a progressive approach is paramount. Runners should gradually increase their mileage, ideally by no more than a couple of kilometres per week, allowing muscles and joints to adapt progressively. Adequate nutritional support is also critical; sufficient intake of carbohydrates and protein is essential for energy and recovery, helping to prevent overuse injuries like stress fractures. Additionally, ensuring adequate calcium and Vitamin D intake supports bone health, further bolstering resilience.

Considering running surfaces can also play a role in injury prevention, especially for new runners or those returning after a break. Evidence suggests that running on softer surfaces like grass reduces impact compared to harder surfaces such as concrete. Incorporating a few weekly runs on grass during the initial stages can aid the body’s adjustment to the new load, contributing to a smoother and safer transition into a regular running routine. Ultimately, the health benefits of running, when approached mindfully, far outweigh potential risks.
