The Adelaide project, once hailed as the nation’s first, crumbled under the weight of escalating costs. A $US49.5 million contract, awarded in 2018 to ProTom International, was terminated by the South Australian Health and Medical Research Institute (SAHMRI) last year, mired in claims of cost overruns. Now, SAHMRI pursues legal action, alleging breach of contract and deceptive conduct, seeking $US32 million in damages. ProTom, for its part, attributes the increased expenses to the pandemic’s ripple effect. This legal tug-of-war leaves the future of the Adelaide unit, and indeed proton therapy in Australia, hanging in the balance.

It’s worth noting the considerable public investment at stake: roughly $115 million committed by state and federal governments. This is a significant sum, and its mismanagement raises legitimate questions about oversight and accountability in major healthcare infrastructure projects.

For families like the Tuckermans, the absence of local proton therapy translates to difficult choices. Billie Tuckerman ultimately traveled to the United States to secure the treatment Evelyn needed, a journey funded by the federal government at a cost of $500,000, plus $40,000 out-of-pocket. Though grateful for the access, she describes the immense emotional and financial toll such a trip exacts on a family, separated and burdened by significant expenses. This underscores a critical inequity: access to potentially life-saving treatment should not depend on a family’s ability to navigate international healthcare systems and absorb substantial costs.

This challenge isn’t just about Adelaide. Federal Health Minister Mark Butler points fingers at past administrations, highlighting a lost opportunity to develop a vital capability. He is now exploring options, suggesting the possibility of relocating the project to another state if a viable plan for South Australia cannot be salvaged. The crucial point, as he states, is that proton therapy should function as a national service, benefiting all Australians.

Australia’s lag in proton therapy adoption contrasts sharply with developments in other developed nations. With over 135 treatment centers operating in the Northern Hemisphere, the Southern Hemisphere’s complete absence is glaring. Some might dismiss proton therapy as a niche treatment, overhyped and excessively expensive. It’s true the technology isn’t a panacea for all cancers. But for certain pediatric cancers and rare tumors, the precision of proton therapy offers a clear clinical advantage, minimizing long-term side effects and improving quality of life. Denying patients access to this option seems shortsighted.

The existing infrastructure in Adelaide—a purpose-built bunker, a project team, and medical staff—arguably gives it a head start. Restarting the project there might prove more efficient than beginning anew elsewhere. Yet, what’s most important is securing a viable plan.

Having witnessed similar healthcare projects stall or fail, it’s evident that success hinges on meticulous planning, transparent procurement processes, and robust risk management. Political will is essential, yes. But without a solid foundation of good governance, even the most well-intentioned initiatives can fall prey to cost overruns, contractual disputes, and ultimately, dashed hopes.

Evelyn, thankfully, is now cancer-free. Still, other Australian families will inevitably face similar diagnoses. Their experiences highlight the urgent need to bridge this gap in cancer care. Proton therapy represents an investment in the future health of Australians, particularly children battling cancer. The time for decisive action is now. Securing access to advanced cancer treatment should be a right, not a privilege dependent on geography or financial means.
